Space Maintainer

Space Maintainers are appliances specially customised by pedodontists or orthodontists in order to maintain spaces in the oral cavity for the permanent teeth to erupt. These spaces in the oral cavity are as a result of premature loss of baby teeth or because of the extraction of baby teeth due to dental decay. Space maintainers ensure that the permanent teeth erupt unhindered in the exact location and alignment in which they are meant to erupt.

Untimely loss or removal of baby teeth may to lead undesirable tooth movements which may increase the chances of developing malocclusions with crowding, crossbites, overjet and overbite. We control this undesirable tooth migration by placing space maintainers in the empty spaces to optimize normal occlusal development after premature loss of baby teeth.

Habit Breaking Appliance

Habit Breaking Appliance is removable or fixed appliance used to stop certain repeated harmful habits in children. It is normal for children to exhibit oral habits like thumb or finger sucking, tongue thrusting or lip biting while growing up. However, if these habits persist beyond the age of four, it can cause severe implications in future.

Continuation of such habits can affect the normal breathing pattern, jaw development and development of normal occlusion. This is exactly where habit breaking appliance comes into role. These appliances stop the habit when everything else has failed. Habit breaking appliances prevent the children to accidently fall back into the habit during day or night time. They help to break the habit and decrease the chances of developing a malocclusion.

Following are some of the commonly observed habits in children:
1) Thumb/Finger Sucking: It is the habit of placing thumb or fingers at varying depths in the mouth. This habit is considered as normal up to 3 to 4 years of age. Persistence of this habit beyond this age may cause malocclusion and poor jaw development. It may cause proclination of the front two upper teeth due to continuous pressure exerted by the thumb or finger. In order to prevent the malocclusion caused by digit sucking, orthodontic appliances like habit crib and bluegrass appliance are used. These appliances are designed to block the thumb and thereby forbid the patient from sucking it.

2) Tongue thrusting: It is the habit in which the tongue makes contact with anterior teeth during swallowing. Constant pressure exerted by tongue thrusting can lead to proclination of the anterior teeth which eventually results in an anterior open bite. In cases of lateral tongue thrust, there are chances of developing a posterior open bite or crossbite. This habit can be intercepted by using appliances like fixed or removable cribs and myofunctional bead, which is used to prevent tongue thrusting habit and helps in promoting proper tongue posture.

3) Mouth Breathing: It is the habit of breathing through the mouth instead of nose. Chronic mouth breathing can cause malocclusion and can result in long, narrow faces and may result in obstructive sleep apnea in later stage. Orthodontic appliances like vestibular screen are used to curb this habit.

4) Lip Biting: It is the habit of biting the lower lip. This often exerts pressure on the lower front teeth, which results in proclined upper teeth and retroclined lower teeth. Habit of biting lower lip can be intercepted using lip bumper which keeps the lip away from the lower teeth, thereby preventing the child to bite his or her own lip.

5) Bruxism: It is the habit of involuntary clenching or gnashing of the teeth. It can either occur when person is awake (awake bruxism) or during the sleep (sleep bruxism). Occasional grinding of teeth is not harmful, but the problem arises when this habit becomes repetitive. Constant grinding of teeth can cause number of problems like pain in jaw ,neck or face, tooth pain or sensitivity, mild headache and wearing away of biting surfaces of the teeth. This habit can be curbed using mouth guards and mouth splints, which are specially customised according to each patient and avoid grinding of the teeth.

Deep Cleaning

Scaling with root planing is termed as deep cleaning. Scaling involves removal of plaque and tartar upto gumline, while root planing is cleaning below gumline using specific dental instruments in order to clean and prevent any bacterial accumulation which otherwise may collect and cause serious gum problems.

Regular cleaning involves removal of plaque and tartar, but it only targets the surfaces of the tooth above the gumline. Gum Surgery

Gum surgery is performed to gain access to the roots of the teeth in cases where non-surgical procedures fail to maintain the periodontal (gum) health. Gum surgeries lift the gums to allow visualization and gain access to the root to eliminate gum infection to restore health, beauty and function of the gums.

Clinical signs for gum surgery are foul odour from mouth; red, swollen or bleeding gums. It is indicated in severe gum diseases, wherein the gums get inflamed up to an extent where they start causing destruction of the bone and tissues surrounding it. Inflammation of gums further leads to the development of pockets or spaces between the gums and the teeth which trap the food and bacteria and cause infection. This eventually causes tooth loss and bone damage.

Sometimes the gum surgery also involves the grafting procedures like:
Bone grafting:This procedure replaces the damaged or destroyed bone surrounding the root of the tooth with new bone.
Tissue grafting: This procedure is used in case of gum recession wherein gum line is noticeably lowered or aesthetically not pleasing.

Splint Therapy

Splinting is the dental procedure used to join two or more loose teeth together in order to evenly distribute the biting forces amongst all teeth. Splinting is used to stabilize mobile teeth and to prevent drifting or tipping. Usually splinting of teeth is done to immobilize mobile teeth following a dental trauma.

Extra Coronal Splints: These are the splints which are bonded to the outer surfaces of the teeth to hold and stabilize the teeth together.
Intra Coronal Splints: These splints function through creation of small channel into the teeth and passing a preformed custom made metallic splint through it, which is then bonded or cemented together in place to hold the teeth together.
Crown Splinting: It permanently fixes the mobile teeth together by crowning all mobile teeth.
Occlusal Splints: Occlusal splints are used to protect the teeth from excessive biting forces in individuals with parafunctional habits like grinding and clenching.

Moreover it is extremely important to take proper care once splinting is completed in order to maintain stability of teeth. Avoid biting too hard on the splint and consume soft diet to prevent any splint breakage. It is also necessary to maintain a good oral hygiene with regular brushing to prevent accumulation of tartar formation near the splints. Contact sports should be avoided until advised.

Guided Tissue and Bone Regeneration

Guided bone and tissue regeneration are the surgical dental procedures which use bone graft material and barrier membranes to initiate the growth of new bone and gingival tissue in places with insufficient bone or gum volumes for proper function and aesthetics.

Guided bone regeneration surgeries like auto transplantation, ridge split and sinus lift is used for the development of hard tissues in the oral cavity. Bone graft is used to increase the height or width of the ridge in cases where it is resorbed. It is mainly used when the jaw ridge gets too narrow to place dental implants.

Guided tissue regeneration
