Nitrous oxide Sedation!

What is Nitrous Oxide?

Nitrous oxide is a colorless and virtually odorless gas with a faint, sweet smell. It is an effective analgesic/anxiolytic agent causing central nervous system depression and euphoria with little effect on the respiratory system.

How is Nitrous Oxide used in Dentistry!

Nitrous oxide (N2O), more commonly referred to as laughing gas, is a mild sedative agent that safely and effectively manages pain and anxiety during dental treatment. The colorless and odorless nitrous oxide is mixed with oxygen and inhaled through a small mask that fits over your nose. Patients are asked to breathe normally and should feel the effects of the laughing gas within minutes.

Contrary to its name, laughing gas does not necessarily make you laugh. The nitrous oxide slows down your nervous system to make you feel less inhibited. You may feel light-headed, tingly, or even heaviness in your arms or legs. Ultimately, you should be calm and comfortable throughout the procedure.

The objectives of nitrous oxide/oxygen inhalation include:-

  1. Decrease or reduce anxiety
  2. Decrease unnecessary movements and reactions towards dental treatment
  3. Patient cooperation and communications becomes better
  4. Threshold reaction of pain becomes better
  5. Increase tolerance for longer appointments
  6. Assist in the care of patients who are medically compromised or mentally or physically disabled
  7. Gagging is reduce
  8. Increase the sedative’s potency.

Indications

  • A patient who is very fearful, anxious, or obstreperous
  • Certain patients with special health care needs
  • A patient whose has severe  gag reflex which interferes with dental care
  • A patient in whom local anaesthesia cannot be obtained
  • A cooperative child who becomes attention deficient due to the lengthy dental procedure.

Contraindications

  • Chronic obstructive pulmonary diseases
  • Severe emotional disturbances or drug-related dependencies.
  • First trimester of pregnancy.
  • Treatment with bleomycin sulphate.
  • Deficiency of Methylenetetrahydofolate reductase

Before giving analgesic/anxiolytic medications to patients with serious underlying medical conditions (such as acute otitis media, recent tympanic membrane graft, sickle cell disease, severe obstructive pulmonary disease, congestive heart failure, or acute severe head injury), the appropriate medical specialists should be consulted whenever possible.

Advantages

  • Administered easily
  • Rapid onset of action
  • N2O has bland, pleasant, non-irritating order
  • Rapid uptake and elimination of N2O
  •  Faster recovery
  • Titration is possible
  • There is a wide margin of safety
  • There is cardio-respiratory stability
  • Nausea and vomiting are not commonly seen
  • Reflex integrity is sustained
  • Patient preparation is usually not required
  • No need for any escort.

Disadvantage

  • Expensive equipment
  • The patient needs to be able to breathe via their nose.
  • Interference between the anterior maxillary injection site and the nasal hood.

Laser assisted periodontal therapy!

As dental technology continues to evolve, so do the available treatment options for addressing existing conditions, such as periodontitis or gum disease. We at Teeth Care Centre, the dentist in Ahmedabad satellite are here committed to staying at the forefront of dental advancements, providing our patients with cutting-edge treatment options that offer the greatest benefits. One such innovation is laser-assisted periodontal therapy, a minimally invasive, highly effective approach to addressing periodontal disease. This state-of-the-art technique allows us to achieve remarkable results with reduced discomfort and faster recovery times compared to traditional surgical methods.

What do you mean by Laser assisted periodontal therapy?

Laser assisted periodontal therapy, also known as LANAP (Laser Assisted New Attachment Procedure), utilizes laser technology to provide gentle and precise cleaning and sterilization of periodontal pockets, promoting tissue reattachment, and supporting the regeneration of bone. This approach differs significantly from traditional periodontal disease therapies, which typically involve surgical procedures to eliminate bacteria and diseased tissue, followed by scaling and root planing. While these conventional methods have proven effective over the years, laser therapy offers patients a more comfortable and less invasive alternative with promising results.

The Advantages of LANAP

Compared to conventional surgical methods like scaling, root planing, and pocket reduction surgery, laser assisted periodontal therapy provides several notable benefits:

  1. Minimally Invasive: LANAP involves using a specialized laser to selectively remove bacteria and diseased tissue within periodontal pockets, leaving healthy tissue unharmed. This precision results in a less invasive procedure, reducing pain and trauma to the surrounding gums and bone.
  • Faster Recovery Times: Since the procedure is minimally invasive, post-treatment discomfort, swelling, and bleeding are significantly reduced. Patients generally experience quicker healing and can return to their daily activities with minimal disruption.
  • Regeneration of Bone and Gum Tissue: Laser therapy promotes the reattachment of gum tissue to the tooth roots and stimulates bone regeneration, a key advantage in restoring oral health.
  • Reduced Risk of Infection: The laser’s sterile environment minimizes the risk of post-treatment infection and helps maintain a healthier mouth during the healing process.

Laser assisted periodontal therapy offers a promising alternative to conventional periodontal surgical methods, harnessing advanced dental technology to provide a gentler and more effective treatment approach. Laser assisted periodontal therapy offers a modern approach to combatting periodontal disease gently and effectively.

‍At Teeth Care Centre, the laser dental clinic in ahmedabad, we are committed to providing our patients with the highest level of care and comfort, utilizing the latest technology and techniques to deliver exceptional results. Dental implant failures!

The replacement of missing teeth by titanium dental implants is currently the gold standard in dental rehabilitation. There are many factors responsible for implant failure they are as follows:-

  1. Habit of Cigarette Smoking:-

Dental implants have a lower survival rate in smokers. Smoking affects the osseointegration process by decreasing the blood flow rate due to increased peripheral resistance and platelet aggregation. The residue of cigarette contains carbon monoxide and cyanide, which delays wound healing capacity. Studies have shown that there is significant marginal bone loss in smokers compared to non-smokers.

  • Oral hygiene and maintenance:-

The accumulation of bacterial plaque leads to disease’s like gingivitis, periodontitis, peri-implantitis. The presence of any type of infection, radiographic evidence like bone loss, and/or neuropathies reduced vascularity concomitant with parallel-oriented collagen fibers may be indicative of a failing implant. Hence it requires maintenance visit for evaluation of the implant and also for the prosthetic component.

  • Medically compromised patients:-
  • Hyperglycemic- affects the ossteointegration of dental implants also it alters the bone matrix, its components, adherence and extracellular matrix.
  • Cardiovascular disorders – it interferes with healing and ossteointegration process, resulting in reduced fibroblast activity, and decreased collagen synthesis. The pathologies like hypertension, atherosclerosis, and congestive cardiac failure.
  • Bleeding disorders-   uncontrolled hemorrhage is caused by platelet disorders, coagulant factors deficiency and using anticoagulant therapy.
  • Peri-implantitis: –

Peri-implantitis is a progressive inflammatory condition which affects the tissues surrounding an osseointegrated implant, leading to the loss of the supporting bone and implant failure. It is characterized by bleeding, suppuration, increased pocket probing depth, mobility of implant, and radiographical bone loss. This inflammatory process is more intense and, goes deeper and faster around the dental implant as compared to the inflammation around the adjacent natural tooth. Systemic antibiotics for Gram-negative anaerobes alter the microbial composition and clinically improve the condition over a 1-year period. The local delivery devices such as Actisite which has fibres containing polymeric tetracycline hydrochloric acid can be used, which significantly reduces total anaerobic bacterial counts.
