When is the surgical removal of wisdom tooth indicated?

Failure of complete eruption of the wisdom tooth, especially in the lower jaw is an extremely frequent occurrence. The treatment includes the surgical removal of the wisdom tooth, also known as the third molar surgery.

What are the indications for Third Molar Surgery?

  • Insufficient space in the jaw for the eruption of third molar.
  • Unerupted third molar, causing pain due to the pressure it exerts.
  • Partial eruption of the third molar, which becomes a site for food lodgement.
  • Grossly decayed third molars.
  • When the third molar is anomalously placed and at times, causes decay of the adjacent tooth. For example: Mesioangular/ Distoangular/ horizontally placed third molars.
  • Periapical Pathology. For example: a cyst/ granuloma.

In all these circumstances, there are signs of pain, swelling and/or inflammation, and hence surgical removal is indicated.

How is the third molar surgically extracted?

Before surgical extraction, a thorough clinical and radiological diagnosis is done. The Medical History of the patient is recorded and required measures are taken.  After proper isolation, anaesthetic agent is employed. In most of the cases, local anaesthesia suffices, but in rare cases, general anaesthesia might be required. This is followed by Flap elevation, creating a gutter by Bone Cutting, elevation of the third molar, Socket compression and lastly taking sutures, which is required in almost all cases. Medications are prescribed and patient is recalled for suture removal and follow-up. During the entire surgical procedure, sterilization is to be maintained. The role of fluoride is to reverse the demineralization of enamel and thus prevent tooth decay.
