Enamel is the hardest inorganic substance in our body but due to some pathological or physiological changes tooth wearing occurs. Tooth wearing is irreversible loss of tooth structure resulting in pain, unlikely look and functional impairment of teeth.

Tooth wear is of FOUR types: –

  1. Abrasion: – It is non carious, mechanical wear of tooth from external forces other than tooth contact. Abrasion is usually due to improper brushing technique (vigorous horizontal tooth brushing), nail biting and pipe smoking.
  2. Erosion: – It is chemical process in which acid dissolves the outer most layer of tooth i.e enamel not originating from bacteria. It can be intrinsic (acid re-flux, vomiting) or extrinsic (dietary habits). Often, high amount of carbonated drinks, sodas and acidic juices leads to erosion of teeth.
  3. Attrition: – It is loss of tooth structure due to tooth – tooth contact. The friction caused due to rubbing of upper and lower teeth leads to reduction in tooth enamel and tooth size. Although it is considered normal process of ageing, but some people have unintentional aggressive rubbing of teeth leading to sensitivity, pulp exposure and pain in Temporomandibular joint.
  4. Ab-fraction: – It is pathological loss of tooth structure due to bio-mechanical loading of tooth resulting in deep notches, wedge shaped or V-shaped near gum area of tooth. This type of tooth wear occurs due to stress and increased occlusal loading during chewing and swallowing.

What oral care should be given to a child in their growing years?

The first tooth appears in baby’s mouth at 6 months of age. This tooth development starts when the baby is inside mother’s womb, so basic care should start from there .The mother should have healthy diet and regular medical check up. Intake of some medicine like tetracycline, phenytoin and increased bilirubin level may discolor the tooth.

The child should be introduced to the dentist at the age of 6 months because the dentist tells how to take good care of baby gums and after each feeding the gums should be wiped off to protect some other infection.

The child’s mother is taught how to help child while brushing, how important it is to inculcate the habit of twice brushing and maintaining oral hygiene. The parents are also told right size of toothbrush to use and how toothpaste helps to avoid decays. Usually parents don’t wean the child from bottle feeding till 4-5 years of age and it results in multiple tooth decays. How Tobacco affects Oral Health?

Tobacco is leading cause of oral cancer especially in south Asian countries. Cigarettes, smokeless tobacco, and other forms of tobacco cause oral cancer, gum disease, and other oral health problems.

Tobacco leads to –

  2. Tooth discoloration.
  3. Increased plaque and tartar.

About 37% of patients who persist in smoking after apparent cure of their cancer will develop second cancers of the mouth, lips, tongue, and throat, compared to only 6% of those who stop smoking.
